Requirements for the Renewal of Black Passports
The applicant must come in person and deposit the following:
- Expired passport booklet.
- Valid photo identification document (such as Dominican identification card, Canadian permanent resident card etc.)
- If the applicant is a minor he/she must present a Birth Certificate issued from the year of 2004 onward and his/her father or mother must present a valid photo identification document (such as passport, Dominican identification card, Canadian permanent resident card etc.)
- Two (2) front photographs, size 2” x 2” and with white background. The ears must be visible and without any earrings on the ears or face.
- Marriage/Divorce Certificate issued from the year of 2004 onward, for name changes (if applicable).
- Passport fee in cash, money order or certified cheque payable to “Dominican Republic Consulate” of USD90.00.
- Fee for delivery purposes to New York City in cash, money order or certified cheque payable to “Dominican Republic Consulate” of USD50.00.
- If you wish to have us mail you your new passport please provide us a Canada Post prepaid express envelope.
Requirements for the Renewal of Blue or Burgundy Passports
The applicant must come in person and deposit the following:
- Expired passport booklet.

Requirements for the Renewal of Passports for Damage, Loss or Theft
The applicant must come in person and deposit the following:
- Damaged passport booklet (if applicable).
- Valid photo identification document (exclusively the Dominican identification card).
- Birth Certificate issued from the year of 2004 onward.
- A declaration under oath with a guarantor of Dominican nationality issued by the Consulate or a police report; the guarantor must present a valid photo identification document (such as passport or Dominican identification card); the declaration fee is USD40.00 payable in cash, money order or certified cheque to “Dominican Republic Consulate”.
- If the applicant is a minor his/her father or mother must present a valid photo identification document (such as passport, Dominican identification card, Canadian permanent resident card etc.)
