Must drop off or mail the following documents:

• Valid Passport or Travel Document.
• Photocopy of all used pages on the Passport or Travel Document.
• Photocopy of your Permanent Resident Card or a photocopy of your valid Canadian Multiple Visa (visitor, worker and student).
• A photograph, the frame size must be 30 mm x 30 mm and stamped indicating date of photograph; photograph must not be older than 6 months. The Photograph must show the full front view of the head, with the face in the middle of the photograph, and include the top of the shoulders.
• Acceptance letter from the Educational Institution.
• Guarantor letter from parent or person who will support you during your stay in the country; attach employment letter and bank statement or bank letter of the guarantor.
• Scholarship document (if applicable).
• Police certificate of good conduct issued by the RCMP.
• Medical certificate with laboratory tests for doping (drugs) and HIV.
• Completed Visa Application Form. ( English | Spanish )
• Photocopy of parent’s authorization letter for minors under the age of 18 traveling alone.
• Bank letter specifying the date the savings or checking account was opened, the annual average or balance maintained in the account, as well as the date of the last deposit.

Processing fee of US $80.00 in money order, bank draft, certified check or cash if in person payable to Consulate General of the Dominican Republic.

When mailing your documents please include a PREPAID COURIER ENVELOPE with your mailing address and phone number.

Note: Student Visa is valid for one (1) year with multiple entries.
